Do you have any pre-existing medical conditions that may affect your ability to participate in strenuous physical activities? (e.g., heart conditions, asthma, diabetes, epilepsy, etc.)?
Have you ever experienced any of the following conditions or symptoms during physical activities? (Check all that apply)
On a scale of 1 to 5, please rate your current level of physical fitness (1 being low, 5 being high)?
Are you physically capable of carrying a backpack and walking for extended periods of time? (For Hiking & Wild Camping)
Are you proficient in swimming? (For Cold Water Swimming)
Are you comfortable with the potential exposure to cold water during the Cold Water Swimming?
